The process of assessment can help to produce a common understanding of needs. The assessor should be any experienced health or social care professional, who normally undertakes assessments as part of their role. The purpose of this aide memoire is to act as a prompt when assessing and reviewing patients with palliative and end of life care needs across conditions.

Definition and Competencies for Key Worker Function

The aim of this paper is to provide a high level definition for Key worker function and competencies.

The key worker function should be an integral part of the role of all relevant Health and Social Care Professionals. The individual who assumes the key worker function will be a member of the patients multidisciplinary team, responsible alongside side other team members, for planning and co-ordinating patient care across interfaces, including within and between care teams and care settings.

The aim of these guidelines is to enable staff within the nursing and residential care home setting, in collaboration with other members of the health and social care team, to provide consistent high quality palliative and end of life to residents and their families.
